We register the deaths of Sri Lankan citizens/dual citizens, under the regulations of the Sri Lanka Consular Functions Act No: 04 of 1981.
The informant of the death should be: Father/Mother/Spouse/Child/Sibling.
Applying at our counter: You can submit your application by visiting us in person, after obtaining an appointment through our website slcgmel.org. You can obtain an appointment on any working day between 9.30am -1.30pm. For public holiday closures please visit here
If you so visit us and submit your application at our counter, and if you submit it within one year from the date of death, the death certificate will be issued by this office and notify you via email.
If you submit the application after one year from the death (even if you submit it at our counter), your application will be forwarded to the Registrar General’s Department for approval and the certificate will be issued upon receipt of the approval.

For the Registration Form B15: A letter of explanation for late registration, addressed to the “Registrar General, Registrar General’s Department, Colombo,” should be attached.
All Photocopies should be submitted in A4 size. Copies of both sides of each certificate/ card should be submitted. For passports; copies of the bio-data page and the observation page should be submitted.
Translations in lieu of originals not accepted.

If the application submitted by post, the relevant fee should be sent by Postal Money Order (Australia Post)/ bank draft drawn in favor of the “Consulate General of Sri Lanka in Melbourne”. The scanned copy of the receipt will be emailed to you once your application is processed.
You must inform this office if there are any alterations/ amendments to be done to the death certificate within 07 days from the date that you received the certificate and return the certificate to this office immediately.
If you have not informed us within 07 days, you must submit the declaration* to this office for the alterations/amendments to be done to the birth certificate along with the relevant documents & fee. Applicant should verify his/her relationship to the deceased.
A request letter signed by you, copy of the death certificate issued by the Australian authorities and documentary proof regarding the required alteration should also be submitted. This may vary from case to case. (ex: If you need to change one of deceased’s parent’s details on the death certificate, you must submit the deceased’s parent’s original birth certificate with correct details)
Your request will be forwarded to the Registrar General’s Department, Sri Lanka and the death certificate will be amended after their approval. You will be notified via email once the amended death certificate is ready to collect.
You must submit the above declaration* to the relevant Divisional Secretariat Office in Sri Lanka along with relevant documents. Your signature on the above declaration and relevant documents could be certified through this office. The documents could only be certified if they have been issued from Sri Lankan authorities and if submitted with originals. Applicant should verify his/her relationship to the deceased.

Please submit a request with details of the death certificate [name of the deceased, date of death, reference no of the application (if available) etc.] via email mail@slcgmel.org prior to visiting this office for your appointment in order to search registries and ready it for collection. You can obtain the copy of the death certificate at our counter after submitting following *application and making the relevant payment. Applicant should verify his/her relationship to the deceased.
We will notify you by an email once the certificate is ready to collect.
You do not need to obtain a prior appointment for collection. You can collect the certificate by visiting this office between 9:15 – 9:30am OR 1:30 – 1:45pm on any working day. Before visiting us, please check our website for any public holiday closures.
If you wish to receive the Certificate via registered post, you can reply us with the name of the deceased, date of death, application reference number and the postal address to send your document. The tracking number will be emailed to you for follow-up.
For collection of the death certificate by a third party, you are required to provide the third party with an authorization letter.
